Introduction
This User Agreement ("Agreement") is made between you ("User," "you," or "your") and Third Party Registry LLC ("Company," "we," "us," or "our"). By accessing or using our website or application ("Site"), you agree to comply with and be bound by this Agreement. If you do not agree to these terms, you must not use the Site.
Eligibility
You must be at least 18 years old and capable of entering into legally binding agreements to use this Site. Additionally, you must be a Healthcare Professional with an active NPI number and must be directly involved in the care of the patient for which data is submitted. By using the Site, you represent and warrant that you meet these eligibility requirements.
Privacy Policy
Our Privacy Policy, which explains how we collect, use, and disclose your information, is incorporated by reference into this Agreement. By using the Site, you agree to our Privacy Policy.
Use of the Site
- Permitted Use: You may use the Site for lawful purposes only. You must not use the Site in a way that violates any applicable laws or regulations.
- Prohibited Use: You are prohibited from:
- Uploading or transmitting any material that infringes any patent, trademark, trade secret, copyright, or other proprietary rights of any party.
- Engaging in any activity that interferes with or disrupts the Site or its servers.
- Attempting to gain unauthorized access to any portion of the Site or any other systems or networks connected to the Site.
- Using any automated means, such as robots or scripts, to access the Site.
User Accounts

Governing Law
This Agreement shall be governed by and construed in accordance with the laws of Oregon in the United States of America, without regard to its conflict of law principles.
